I, the undersigned parents or guardians, hereby apply to enroll the camper whose name appears on the reverse, in Gold Arrow Camp. In doing so, I acknowledge that we have read and that agree to each of the following conditions:

1. Tuition/Refund Policy.
Tuition is refundable less a $150 processing and cancellation fee if, and only if, the camper's enrollment is cancelled in writing before February 1. A fee of $250 will apply if the cancellation is received before March 1, and a fee of $350 if the cancellation is received before May 1. No refunds will be given for cancellations received after May 1. A single exception is made for campers who suffer physical illness or injury on or after May 1 and whose attending physician certifies in writing that the illness or injury will not allow the camper to safely participate in any offered camp session. In this event, we will refund the camper's tuition less a cancellation fee of $350.

Campers who arrive at camp late or leave early will receive pro-rata refunds only when it is the result of the camper's physical injury or illness, certified by his or her physician. Homesickness and dismissal from camp are not grounds for refund.

2. Health.
In the event of a medical or surgical emergency, I hereby give permission to any physician selected by the camp director, to secure proper treatment, which may include hospitalization, if necessary, for the registered child. I understand that Gold Arrow Camp will make all reasonable efforts to contact us should an emergency arise. I will keep the camp director advised should I be away from my normal residence or place of employment for any extended period of time during the camp season. I understand I will receive a health form via U.S. mail, which must be completed and received by Gold Arrow Camp prior to the camper's arrival at camp.

3. Waivers of liability.
a. I agree that Gold Arrow is not responsible for the loss or damage of campers' personal belongings by fire, theft, laundry etc. I understand that Gold Arrow Camp recommends campers not bring expensive items such as watches, cameras, radios, portable stereos, etc.
b. I understand and acknowledge that certain camp activities, whether or not listed in the brochure, which include but are not limited to sailing, water skiing, horseback riding, etc. have an increased risk of injury associated with them. I hereby release and agree to indemnify and hold harmless, Gold Arrow Camp and all its officers, directors, employees, agents and representatives whatsoever from any and all losses, claims, damages, liabilities, costs and expenses including attorneys fees, which they or any of them, or camper may sustain or incur in any way arising out of or in connection with camper's participation in any and all camp activities.

4. General Matters.
a. I agree that the camper and his/her parents and relatives will abide by the rules and regulations set by the camp for the health, safety, and welfare of the camper.
b. I understand that the camp encourages frequent letter writing, and that due to the camp's limited telephone service, phone calls to campers are allowed only in emergencies.
c. I agree that the camp reserves the right to cancel, change, or substitute programs or activities as listed in its brochure, video and CD-ROM, when necessary during summer sessions.
d. I consent to the use of any pictures of the camper in connection with the camp's future advertising or promotion.
e. I consent to your distributing campers' addresses to other camper families.
f. I understand that a number of the camper's activities will occur at the waterfront and involve swimming and various water sports. I certify to you that the camper can swim adequately to participate safely in such activities, and that the camper suffers from no physical or medical condition which would make his/her participation unsafe or unusually dangerous to the camper or others.

5. Dismissal From Camp
I understand that the camp reserves the right to dismiss a camper whose conduct is dangerous, illegal, or at the discretion of the camp director, detrimental to the camp and/or to other campers, otherwise unsatisfactory, or as stated in the following Camper Standards of Behavior and Appearance:

Gold Arrow Campers are expected to treat other campers and staff with respect at all times, and value each camper's right to have a fun experience.

The following behaviors are not permitted at Gold Arrow Camp and will result in camper being dismissed from camp without any tuition refund:
1. Teasing, taunting, threatening or discriminating comments, or harassment or hazing of any kind.
2. Violence of any kind.
3. Use of foul language or discussion of inappropriate topics as determined by the Camp Directors.
4. Possession or use of tobacco, alcohol, or illegal drugs. All prescription and over the counter medications must be stored in the infirmary and administered under the supervision of our camp Health Personnel.
5. Sexual or intimate relationships.
6. Unauthorized absence from cabin or activity.
7. Abusive or disrespectful behavior towards any member of the camp community.
8. Inappropriate appearance or attire (see below).
9. Possession or use of personal electronic equipment (cell phones, electronic games, portable stereos, etc.).

Appearance and Attire. Campers should bring to camp clothing that is appropriate for an outdoor, active camp program. Use of make up and/or wearing of expensive or trendy clothing are discouraged while at camp.

The following are not allowed at camp:
1. Clothing with any logos or graphics depicting violence, bands who play violent music, drugs alcohol, or inappropriate language or topics.
2. Jewelry that includes collars, chains, and face or body piercings. Post earrings (worn on ear lobes) are allowed.
3. Clothing that does not cover underwear. Clothing must fit closely enough so that campers' clothing do not drag or get caught in belay cables, reins, etc. We encourage female campers to wear one-piece swimsuits when participating in water sports. String bikinis and thong bathing suits are not permitted. No midriff or cleavage- baring t-shirts or tops are allowed. Pants and shorts may not be rolled down and must cover camper's bottom completely at all times (including while seated).

Note: We will strictly enforce our dress code. To avoid being required to mail additional clothing to camp, we ask that parents supervise packing and make sure that no inappropriate clothing items are brought to camp.

I agree that there will be no refund in the event of dismissal.
